Notice of Denial. If a claim is wholly or partially denied, the denial notice shall contain # the reason or reasons for denial of the claim, and # references to the pertinent Plan provisions upon which the denial is based. Unless the claim is submitted for arbitration as provided below and in the Plan, the Claims Administrator’s decision or action shall be final, conclusive and binding on all persons having any interest in the Plan.

(d) Notice of Decision. If the Plan Administrator affirms the denial of part or the entire claim, the Plan Administrator shall notify the claimant in writing of such denial. The Plan Administrator shall write the notification in a manner calculated to be understood by the claimant. The notification shall set forth the specific reason for the denial and a reference to the specific provisions of the Plan on which the denial is based.

If a Benefit Claim is Denied, the Administrator shall provide the claimant with written or electronic notice containing # the specific reasons for the Denial, # references to the applicable Plan provisions on which the Denial is based, # a description of any additional material or information needed and why such material or information is necessary, and # a description of the applicable review process and time limits.

(c) Notice of Decision. If the Plan Administrator denies part or all of the claim, the Plan Administrator shall notify the claimant in writing of such denial. The Plan Administrator shall write the notification in a manner calculated to be understood by the claimant. The notification shall set forth:
