Acknowledgement. Executive affirms that she has read this Agreement and been advised that she has twenty-one (21) days from the date she received it to sign this Agreement, and that she has been advised in writing to consult with an attorney prior to signing this Agreement. Executive affirms that the provisions of this Agreement are understandable to her and she has entered into this Agreement freely and voluntarily.

By executing the Agreement below, Executive confirms and acknowledges that she has reviewed the information about the offer described above and given to her as part of this Agreement. Executive further acknowledges that she has been granted twenty-one (21) days from the date she received this Agreement within which to consider this Agreement. Executive further acknowledges that by virtue of being presented with this Agreement, she is hereby advised in writing to consult with legal counsel prior to executing this Agreement. Executive acknowledges that if she executes this Agreement prior to the expiration of twenty-one (21) days, or chooses to forgo the advice of legal counsel, she has done so freely and knowingly, and she waives any and all future claims that such action or actions would affect the validity of this Agreement. Executive acknowledges that any changes made to this Agreement after its first presentation to her, whether material or immaterial, do not re-start the tolling of this twenty-one (21) day period.
Executive is hereby advised, and Executive acknowledges that he has been so advised, to consult with an attorney of Executives choice before signing this Agreement. Executive acknowledges that he is signing this Agreement (even if done before the expiration of twenty-one (21) days) after having the opportunity to consult with an attorney and to consider the terms of the Agreement for at least twenty-one (21) days; that Executive has carefully read this Agreement in its entirety; that Executive has had an adequate opportunity to consider it; that Executive understands its terms; that Executive voluntarily assents to all the terms and conditions contained herein; that Executive is signing it voluntarily and of Executives own free will, and that Executive is not suffering from any disability or condition that would render Executive unable to enter into this Agreement.

Executive acknowledges that Executive has [twenty-one (21)][forty-five (45)] days to consider this Release Agreement (but may sign it at any time beforehand if Executive so desires), and that Executive is advised to consult an attorney in doing so. Executive hereby acknowledges that Executive understands the significance of this Release Agreement, and represents that the terms of this Release Agreement are fully understood and voluntarily accepted by Executive. Executive also acknowledges that Executive can revoke this Release Agreement within seven (7) days of signing it by sending a letter to that effect at the following address:

Executive acknowledges that the Company has advised him in writing to consult with an attorney of his choice before signing this Agreement, and that Executive has been given the opportunity to consult with an attorney of his choice before signing this Agreement.
Acceptance. Executive agrees and accepts this Agreement. Executive acknowledges that he has not signed this Agreement relying on anything not set out herein. Executive acknowledges that if he is signing this before , he has decided not to wait for the full twenty-one (21) day period, even though he has the right to do so.
